You need to fill this form out and fax it back to LANgrafix to start processing your order. Make sure all fields are filled out. If you plan on paying by Credit Card, you must include the billing address and security PIN located on the back of the card. Also, remember to specify what mode of shipping (Next Day, 2nd Day, Ground) you wish to use. All these fields are important to ensure proper processing of your order.
